This time the damage was not only spacing: some sentences had lost words at the page edge, a few explanations had the next question's text glued on, and several explanations of the wrong options had been torn off by the scanner — those were rewritten. Reading them also turned up about twenty explanations that stated something wrong, for example that tertian malaria fever comes every 48–72 hours (it is every 48) or that strychnine acts on GABA receptors (it blocks glycine receptors). In one large block of the bank — about a thousand questions from recalled papers — the correct answer landed on option (b) roughly 47% of the time and on (d) only 8%, which means someone answering (b) to everything would have scored far better than chance and their practice score would have been flattering them. No individual answer was wrong; the pattern was in the set. The options on 953 of those questions have been reshuffled so the answer now falls evenly across (a), (b), (c) and (d). Eleven have been rewritten properly, and four of those needed the question itself repaired, including one whose premise was simply false. Three could not be rewritten because the question was wrong at the source rather than badly explained: one of them dated a medical declaration to the wrong subject entirely, and another offered four fluids for a situation in which none of the four is correct. A 13-year-old boy with one-sided nasal blockage and repeated nosebleeds over six months is the classic description of a juvenile nasopharyngeal angiofibroma, and that was offered as an option — but the stored answer was antrochoanal polyp, and the explanation had been written to defend it by claiming those polyps commonly bleed in teenage boys. They do not; that is exactly the point the question was testing. It has been taken down. Eighteen more questions were withdrawn in the same pass: some where two options were equally defensible as written, some where the question had lost the words that carried it, and five that were not really questions at all — three of them named their own answer in the question text. | ✅ fixed |
| 2026-09-08 | Question bank | Explanations that pointed at the wrong option letter. In some questions the "other options" section had labels shifted by one, so a paragraph headed "Option B" would describe what was actually option C. Ten questions had this, and in eight of them the mislabelled paragraph landed on the correct answer itself — so a student was told the answer was B and then read a paragraph headed "Option B" describing something else entirely. All ten are corrected. No answer key was changed by this: the answers were right, the labels were not. The whole written bank has now been checked for this, which had never been done before. | ✅ fixed |
| 2026-09-08 | Question bank | More questions pointing at a picture that is not there. An earlier entry on this page said no question that mentions an image now ships without one. That was true of the check that had been run, and the check turned out to have a gap — it was not looking for the words ultrasound, MRI, mammogram or barium study at all, or for phrasings like "the swelling shown below". Twenty-five more were found. Most are answerable from the case description and only the dangling sentence has been removed, or the finding written into the question where it was previously only in the picture. | 2026-09-06 | Question bank | Reported by a user: a question asked about "the instrument shown here" and no picture was ever attached. It was right, so that question has been taken down. Every other live question was then checked for the same fault - a question that points at an image, a scan or a photograph it does not actually show. Most turned out to be answerable from the case description alone and were left in place, but two more genuinely could not be answered without the missing picture and have been taken down as well. One was repaired instead of removed: a "identify this national health programme logo" question whose photograph was found and put back, so it now shows the emblem it always should have. Two further questions were being served as picture questions whose figure had never finished uploading; both are fixed. No answer key was changed. | ✅ fixed |
| 2026-09-06 | Question bank | Garbled letters in the explanation text of some questions have been corrected. In one of the source books a font problem had replaced certain characters with a capital I, so words came out as "Ixation" instead of "fixation", "gramInegative" instead of "gram-negative", and names lost their accents. The most important of these were roman numerals: "antithrombin Ill", "CN Ill" and "Type Ill" should all have read III. Every affected word was read in context and corrected by hand rather than by find-and-replace, because a few lookalikes were correct as printed and had to be left alone. A handful of questions appeared twice in the bank - the same stem and the same options, usually because the same question was reprinted in two different years' papers - so it was possible to be served one you had already answered. Two pairs that look identical in text were deliberately LEFT IN: they are picture questions whose stems read the same but whose images, and answers, are different. | ✅ fixed |

The /review command was serving an ordinary practice question instead of your review queue - only the button did the right thing. And stepping out of review into Tutor Mode left the old "Next review" button underneath, so the buttons disagreed with the questions you were being given. Checked, and it was a real gap in the original source, not something that got lost along the way. Scanned every live question for the same pattern (options pointing at a numbered or lettered list that never appears) and found three more the same way. | 2026-08-28 | Question bank | 449 more questions now carry a written explanation, taking the bank from about 3,900 to 4,400 servable. Every one was checked by independently re-solving it and confirming the answer matched what's stored before the explanation was written — the key itself was never touched. About 70 questions from the same source turned out to be unanswerable as extracted (the printed original referenced a list or a data table that never made it into our copy); those stay held back rather than being explained around, and are now specifically tagged so a future pass through the original books can find all of them at once instead of rediscovering them one at a time. | ✅ fixed |

These are the ones that had been held out of the bank because they could not be rebuilt honestly — a question whose entire text was "Assessment of resources.", one with four clean options and no question above them, one where a head-injury case had been fused onto four options about urinary chemistry by a bad scan. Every deleted row was written to a file first and read back and compared before anything was removed, and they were taken out of the import files too so a future re-import cannot quietly put them back. The number of questions you can actually practise did not change: none of these was ever being served. | ✅ fixed |
| 2026-08-27 | Question bank | Four answer keys were corrected and the questions put back into the bank. A newborn galactosaemia question was keyed to the wrong enzyme; a forensic question cited the wrong section of the law; a milestones question was keyed "all of the above" when only one of its four statements is true; and a breast cancer staging question was keyed to a stage its own printed staging table contradicts. All four had been held back rather than served while the keys were in doubt, so nobody was ever shown the wrong answer. One more question stays held: it asks which type of gastric ulcer goes with low stomach acid and two of its four options are correct, which no answer key can fix. | ✅ fixed |

It avoided repeating a question you had recently seen, but it never looked at the SUBJECT, so the same subject could come up two or three times in a row — measured at roughly one back-to-back repeat every fifteen questions. Tutor Mode now holds back the last three subjects you were served, so the feed moves through all 19. Picking a subject deliberately still keeps you in that subject. | ✅ fixed |
| 2026-08-27 | Bot | Exam Mode has a new Custom module. Tick any set of subjects and it builds a timed paper from just those. The questions are split evenly across what you tick rather than by how many the bank happens to hold, so pairing a small subject with a large one gives you half of each instead of a paper that is almost entirely the large one. Between drilling a single subject and sitting the whole syllabus, there was nothing — this is that middle. | ✅ fixed |
| 2026-08-27 | Question bank | Four questions were carrying an advertisement for someone else's app, printed into the question text by the scanner along with the rest of the page. They were live. A search of the whole bank for anything of that kind — app names, links, channel invitations — found those four and nothing else, and all four are clean now. Ten went back in, including one whose picture turned out to be a photograph of an entire book page with two unrelated questions in the margin, now cropped to the instrument it was asking about. Three were pulled because their own stated answer disagrees with the reference table printed inside the same question, and those stay out until I can check them against the original paper. The remaining thirty stay out: most of them are missing the question itself, not just an explanation, and a question I cannot rebuild honestly is one I will not serve. | ✅ fixed |
| 2026-08-20 | Grand Tests | You can now add two extra Grand Tests to an active plan for ₹99, instead of having to buy another whole month to get more mock sittings. It is offered only when you have actually used up the tests your plan includes, it adds tests and nothing else — no extra days, no new plan, nothing renews — and the tests stay valid for 30 days. Because it can only be bought on top of a plan you are already using, it is not covered by the cooling-off refund window; if the tests fail to arrive or do not work, that is still refunded in full. Each was written after looking at that question's own figure, and each says why the other options are wrong rather than only why the answer is right. That one report led to a check of the whole bank, which found 30 more questions where the parser had merged the NEXT question's text into an answer option. 20 of those are now repaired — including one where an option had swallowed a second answer, so restoring it gave the question its missing fourth choice back. 13 are held back until I can rebuild them properly, and the question that was flagged is out of circulation until its figure is in place. | ✅ fixed |
